That is exactly the information they need to have. Just be sure to keep your addresses up to date.
No, that’s right – they need to be able to distinguish you from all the other people out there that have the same name, and possibly the same date of birth.
That is standard information that the company will request. The companies have a change of beneficiary form that is usually completed by the owner of the policy in order to make the change.
